Cefuroxime axetil
Indications/Uses
Treatment of upper & lower resp tract, nose & throat, GUT, skin & soft tissue infections; gonorrhea, acute gonococcal urethritis & cervicitis; early Lyme disease; subsequent prevention of late Lyme disease in adults & childn.
Dosage/Direction for Use
Adult Most infections 250 mg bid. UTI 125 mg bid. Mild to moderate lower resp tract infections eg, bronchitis 250 mg bid. More severe lower resp tract infections or if pneumonia is suspected 500 mg bid. Pyelonephritis 250 mg bid. Uncomplicated gonorrhea 1 g as a single dose. Usual duration of therapy: 7 days (range: 5-10 days). Adult & childn >12 yr Lyme disease 500 mg bid for 20 days. Childn Most infections 125 mg (1 x 125 mg tab) bid to a max of 250 mg daily. Childn ≥2 yr w/ otitis media or where appropriate w/ more severe infections 250 mg (1 x 250 mg tab or 2 x 125 mg tab) bid to a max of 500 mg daily.
Administration
Should be taken with food: Take w/ meals for optimum absorption.
Contraindications
Hypersensitivity.
Special Precautions
Allergic reaction to penicillins or other β-lactams. Prolonged use may result in the overgrowth of other nonsusceptible organisms (eg, Candida, Enterococci, Clostridium difficile). Consider diagnosis of pseudomembranous colitis in patients who develop serious diarrhea. Direct Jarisch-Herxheimer reaction following Lyme disease treatment. May impair ability to drive or operate machinery. Pregnancy & lactation.
Adverse Reactions
Overgrowth of Candida from prolonged use; eosinophilia; headache, dizziness; GI disturbances including diarrhea, nausea, abdominal pain; transient increases in hepatic enzyme levels (ALT, AST, LDH); erythema multiforme, Stevens-Johnson syndrome, toxic epidermal necrolysis (exanthematic necrolysis).
Drug Interactions
May lower bioavailability w/ drugs which reduce gastric acidity. May lower estrogen reabsorption & reduced efficacy of combined OCs. Use of glucose oxidase or hexokinase methods to determine blood/plasma levels is recommended as false -ve result in ferricyanide test may occur. +ve Coombs' test may interfere w/ cross-matching of blood.
